Let-off for nurses after deadly drugs mix-up

Two bungling nurses who gave a 93-year-old war veteran a fatal dose of the wrong drugs are set to be let off with cautions. Staff at the Diana, Princess of Wales Hospital in Grimsby took more than an hour to realise former marine Robert Welch had collapsed on 30 June last year.
