Doctors repeatedly ignored warnings about a dangerously disturbed mental patient before he hacked his flatmate’s face apart with a meat cleaver, a court heard. Tomas Bouska, 34, cracked open Nicholas Hill’s skull and left his nose hanging off during the savage attack.
