Nurse smoked crack cocaine with patient

  A nurse who took crack cocaine and heroin with a patient after asking him to buy her drugs has been struck off. Alison Fisher also failed to report a colleague who was having a relationship with the same man, the Nursing and Midwifery Council heard.
