Prison nurses failed to cut ligature when they found man hanging

  Two prison nurses who found an inmate hanging in his cell failed to check for signs of life or cut the bed sheet around his neck, a hearing was told. Oluwaseun Kadiri and Rafiu Oladejo allegedly failed to check on Mark Smith, 39, then filled in reports claiming they had check him twice at HMP Thameside in Woolwich, southeast London.
