Boss fined after this scaffolder fell to his death

The owner of a scaffolding company was breaching  health and safety regulations when a worker fell to his death, a court heard. David Smith, 38, fell from more than 16 feet onto the concrete when the scaffold he was dismantling at a west London site collapsed under him on 16 February 2017.
