The committee later heard that the death rate of child heart patients reached a peak of two in three at the height of the scandal.
At best, just 39 per cent of babies survived cardiac surgery.
Miss Foster told the hearing: ‘Too many babies were dying, whatever the complications they presented.
‘
But she added that Dhasmana was aware of the problems and had tried to stem the tide of fatalities.
Miss Foster added: ‘It was not suggested Mr Dhasmana was not self-critical.
The evidence is he was continually searching his conscience.
‘He attended the post mortems and was wracked with self-doubt at times.
‘He tried to review his results – sadly that analysis was flawed.
‘
The hearing continues tomorrow.
